AKR1B10 is a secretory protein that interferes with cell survival by modulating lipid synthesis, mitochondrial function, and oxidative status. AKR1B10 may promote carcinogenesis through the conversion of highly reactive aldehyde and ketone groups into hydroxyl groups. Overexpression of AKR1B10 is an initial event in the carcinogenesis of various human malignancies, including non-small cell lung carcinoma (NSCLC), pancreatic cancer (PC), breast cancer, oral squamous cell carcinoma (OSCC), nasopharyngeal carcinoma (NPC) and hepatocellular carcinoma (HCC).

Bioinformatic and structural analyses proved that in the AKR1B10 primary structure Cys299 represents the Cys298 homolog of AKR1B1 which may play a significant role in carbonyl reducing activity of AKR1B10. Due to this conserved Cys299 residue, AKR1B10 may be equivalently inhibited by fibrates.
Based on its overexpression in neoplastic cells, AKR1B10 is considered a tumor marker that can be detected in situ and the blood.
