Homocysteine is one kind of thiol amino acid which plays an important role in methionine metabolism. Methionine can be activated to form S-adenosylmethionine (SAM) with the help of ATP. The SAM, reacting with protein arginine methyltransferases which can produce methylarginines, serves as the donor for methyltransferases. SAH is the metabolic precursor of nearly all homocysteine which can be mainly produced via SAM-dependent methyl transfer reactions. Moreover, SAH has the ability to form homocysteine according to the hydrolysis. In previous research, the ratio of SAM: SAH is used as the indicator for the potential of cellular methylation as SAM and SAH are necessary substrate and product for the methyltransferase reactions. In recent years, SAH has been found to be a high-efficient biomarker for the association between chronic kidney disease (CKD) and cardiovascular disease (CVD).

CKD is a type of kidney disease which has no symptoms in the early stage but loses the kidney function gradually over a period of months and years.
