Background of Anti-acetylcholine Receptor (AChR) Antibody

Mammalian nicotinic acetylcholine receptors (nAChRs) are composed of five subunits arranged around a water-filled pore. Three types of autoantibodies against AChR of skeletal muscle are detectable in patients with myasthenia gravis (MG) including binding, blocking, and modulating anti-AChR autoantibodies. Modulating autoantibodies correlate best with the severity of the disease, but are also technically most difficult to measure because the assay generally requires fresh human muscle cells.

The Role of Anti-AChR Antibody in Myasthenia Gravis (MG)

MG is an autoimmune disease limited to specific organs in which autoantibodies attack nAChRs in neuromuscular junctions. A diagnosis of MG is based on pathognomonic history, clinical characteristics, and the results of necessary examinations. In cases of mild symptoms or questionable examination results, diagnosis relies more on pathognomonic history. Thus, it is difficult to diagnose patients whose medical histories have been poorly recorded. Normally, the repetitive nerve stimulation test, the edrophonium test, anti-AChR autoantibody titers are used for MG diagnosis. It is worth mentioning that an abnormal increase in anti-AChR autoantibodies titer is observed in more than 90% of cases of MG. Now, anti-AChR autoantibodies have become an ideal biomarker of MG with superior diagnostic accuracy, and it is widely-accepted that antigen-specific apheresis of anti-AChR autoantibodies seems to be a safe and effective treatment for MG that can be scaled up for clinical testing.

Anti-AChR Autoantibody Detection

A solid-phase enzyme immunoassay for the measurement of anti-AChR autoantibodies in the sera of patients with MG is reported and the correlation between antibody titers measured by enzyme immunoassay and the immunoprecipitation assay was significant.
