Background of Anti-Adrenal Autoantibodies

Anti-adrenal autoantibodies, also named as adrenal cortex antibodies, are a group of autoantibodies that specifically target autoantigens on the adrenal cortex. They can lead to several renal disorders or autoimmune diseases, such as autoimmune adrenalitis, autoimmune polyglandular syndromes, and other related diseases. Antibodies against steroid 21-hydroxylases, steroid 17 alpha-hydroxylase (P450c17, 17 alpha-OH) and cytochrome P450 side-chain cleavage enzyme (P450scc) are all anti- adrenal autoantibodies that react with primate adrenal cortex and cause dysfunctions of adrenal biosynthesis.

The Role of Anti-Adrenal Autoantibodies in Adrenalitis

Adrenalitis, also known as adrenitis, is a type of adrenal glands inflammation with insufficiency of adrenaline or noradrenaline. Autoimmune adrenalitis is the most common of adrenalitis that often accompanied by other autoimmune diseases and the most frequent cause of Addison's disease. Autoimmune destruction, impaired steroidogenesis or adrenal destruction can result in adrenal insufficiency of autoimmune adrenalitis. Patients with early stages or focal adrenalitis are usually asymptomatic, but more than 90 % of the adrenal cortex is destroyed when clinical manifestations appear. Therefore, early biomarker researches are necessary to improve the diagnosis of adrenalitis.

Autoimmune adrenalitis is a kind of adrenalitis characterized by the presence of several anti-adrenal autoantibodies including anti-21-hydroxylases, anti-17 alpha-OH, and anti-P450scc, which were first identified in the serum of patients with Addison's disease in 1957. These anti-adrenal autoantibodies mainly are IgG1, IgG2, and IgG4 subclasses. The detection of high levels of adrenal cortex antibodies on cytostatic sections of adrenal glands by immunofluorescence has been the markers for autoimmune adrenalitis diagnosis for many years.
