Background of Anti-TPO Antibodies

Thyroid peroxidase (TPO) is a membrane-bound, glycosylated, heme-containing protein which contributes to thyroid hormone synthesis. It can catalyze the oxidation of iodide on tyrosine residues in thyroglobulin for the synthesis of triiodothyronine and thyroxine (tetraiodothyronine). TPO is one of the most important thyroid gland antigens and is located on the apical membranes of the thyroid follicular cells. Anti-TPO antibodies are a type of anti-thyroid autoantibodies targeted against TPO in the blood, which are the most clinically relevant anti-thyroid autoantibodies. Autoantibodies to TPO are most commonly associated with Hashimoto's thyroiditis. In addition, anti-TPO antibodies can activate complement system and are considered to be significantly involved in thyroid dysfunction and the pathogenesis of hypothyroidism.

The Role of Anti-TPO Antibodies in NAA Associated Thyroiditis

Thyroiditis is the inflammation of the thyroid gland that is located on the front of the neck below the laryngeal prominence and produces hormones to mediate a variety of metabolism pathways. Anti-TPO antibodies are specific to TPO autoantigens. Compared with linear epitopes, most of the antibodies produced target the conformational epitopes of the immunogenic carboxyl terminal region of TPO protein. Actually, most of anti-TPO antibodies are produced by thyroid infiltrating lymphocytes, with minor contributions from lymph nodes and the bone marrow. The thyroid cell damage attributes to complement activation and antibody-dependent cell cytotoxicity. Levels of anti-TPO antibodies are increased in greater than 90% of patients with active autoimmune thyroiditis. Statistical data show that anti-TPO antibodies are the most common anti-thyroid autoantibody, located in approximately 95% of patients with chronic lymphocytic thyroiditis, 75% of Graves' disease and 10-20% of nodular goitre or thyroid carcinoma. Therefore, the presence of anti-TPO antibodies is an indication for treatment in those patients with early/ compensated hypothyroidism.
