Opioid dependence is a serious health problem in the United States and around the world. Even with approved drugs to treat the disease, the relapse rate remains high. Vaccines designed against abuse drugs use a three-component formula: haptens, conjugate proteins and adjuvants. The opioid itself is not immunogenic, so the opioid analog must be chemically linked, that is, conjugated to an immunogenic carrier protein to produce an antigenic motif. The immunoconjugate is then combined with an adjuvant or immunostimulatory agent to form a vaccine.

Mechanism of Action of Opioid Vaccine

The method of developing a drug addiction vaccine is different from the method used in other drug development. Although the opioidergic treatment agents are designed to target opioidergic effector systems in the brain through pharmacodynamic mechanisms, vaccines act as “pharmacokinetic” antagonists. After administration, the vaccine stimulates the production of a drug-specific antibody that binds to the drug in the circulating blood and extracellular fluid when the drug is ingested. This effect reduces or slows the distribution of the drug to the brain and theoretically attenuates the drug's reinforcing or addictive effects.
