Mycoplasma hominis (M. hominis), belonging to the Mollicutes class in the Mycoplasmataceae family, is a fastidious bacterium, which usually colonizes the lower urogenital tract and may cause systemic infections in neonates and genital infections in adults. It can also be the cause of serious extra-genital infections, which occur primarily in immunosuppressed or predisposed subjects. In women, infections are associated with severe pregnancy and postpartum complications, including abortion, endometritis, preterm birth, and low birth weight. Because M. hominis is not visualized on Gram staining and shows slow growth on blood agar compared to general bacteria, its detection has occasionally been challenging. Moreover, M. hominis is resistant to beta-lactam antimicrobials that are often used in empyema. To date, the molecular mechanisms of the pathogenic and virulence effects of M. hominis have not been well understood and require further investigation.

There are many specific proteins on the surface of M. hominis, such as P50, P60, P80, and P100, which play an important role in the attachment of bacteria to host cells, an important step in bacterial colonization and survival in human genital tracts. It has been shown that M. hominis alters the expression of genetic diversity adhesion proteins by truncation and/or by reversible frameshift mutations, which probably provide better conditions for the attachment to different receptors in the human body and lead to extra-genital infection such as arthritis, brain abscess, endocarditis, pneumonia, and wound infection. In addition, pathogens expand infection and escape the host immune system through surface antigenic variation, therefore, immunoassay methods for detection of antibodies against M. hominis in humans are usually based on surface antigens. However, the diagnosis and prevention of human infections are difficult due to the extensive variation in the genome and antigenic structure. One way to solve this problem is to target constant and specific sequences of more than one surface protein and use these fragments in chimeric proteins to increase the likelihood of detection of specific antibodies against M. hominis in human serum or plasma.
