Introduction of Cholinesterase (ChE) Marker

Cholinesterase (ChE) is a member of the cholinesterases family which is found in nerve endings as well as in erythrocytes and other tissues. There are two types of cholinesterase: acetylcholinesterase (ACHE) and butyrylcholinesterase (BCHE) with the former hydrolyzes acetylcholine more quickly and the latter hydrolyzes butyrylcholine more quickly. The term cholinesterase is usually used to refer specifically to butyrylcholinesterase which is synthesized in the liver and thus can be an accurate reflection of liver function. In addition, serum cholinesterase is closely associated with the synthesis of serum albumin and has many diagnostic applications. Importantly, the level of cholinesterase may be reduced in patients with the advanced liver disease.

Cholinesterase (ChE) Marker of Liver Cirrhosis

In the past years, studies have suggested that the level of serum ChE is able to reflect liver function adequacy in various clinical situations. For example, in familial intrahepatic cholestasis, following successful external biliary diversion, increasing levels of serum ChE have been shown to be a marker of successful surgery. As a unique enemy that is exclusively synthesized by the liver like albumin and coagulation factors, serum ChE level of patients with cirrhosis is significantly lower than that of healthy people. What’s more, it has also been reported that patients with decompensated cirrhosis had significantly lower levels of serum ChE compared to well-compensated cirrhosis. In this case, cholinesterase can be used as an excellent biomarker of cirrhosis with good sensitivity and specificity.
