Kidney transplant biopsy 12 weeks after ABO-incompatible kidney transplantation.

The case report discusses a severe antibody-mediated rejection (AMR) after ABO-incompatible kidney transplantation complicated by Serratia marcescens sepsis. A 58-year-old woman received an ABO-incompatible transplant, with initial anti-A titers reduced to safe levels. However, 12 weeks post-transplant, her anti-A IgM titers spiked to over 5000 following sepsis, leading to fulminant AMR. Experiments hypothesized bacterial epitopes shared with blood group A antigens, although direct interaction was not demonstrated.
The significance of this research lies in highlighting the potential for Gram-negative infections to trigger late AMR in ABO-incompatible transplants.
