Eye is a susceptible organ to various diseases of inflammatory, infectious or degenerative nature, which can cause discomfort, visual disturbance, vision impairment, or even blindness. In the anterior segment of eye, ocular diseases include conjunctivitis, dry eye disease, keratitis, ulcer, cornea neovascularisation, keratoconus, cystinosis, neovascular age-related macular degeneration (AMD), diabetic retinopathy, glaucoma, hereditary retinal degenerations, retinitis, posterior uveitis and eye tumors often occur in posterior eye segment. Generally, some of these symptoms are very frequent and are associated with population aging. Besides, lifestyle also is involved in ocular diseases, such as the increase of diabetes prevalence will lead to diabetic retinopathy. Lastly, environmental pollution and the continuous use of digital devices are also considered to affect the prevalence of anterior segment eye diseases such as dry eye syndrome.

Delivery System Targeting Eye

Although eye drops are the most accessible formulations during the treatment of ocular anterior diseases, this administration method presents poor bioavailability since the corneal barrier and pre-corneal factors. Over the past decades, numerous feasible targeted delivery systems have been designed for ocular anterior disease therapy. For example, flurbiprofen-loaded PLGA nanoparticles have shown a controllable release profile in vitro. This therapy approach presented an improved anti-inflammatory effect compared with commercial flurbiprofen eye drops. Besides, a micellar supramolecular hydrogel was developed with methoxy poly block polymer and α-cyclodextrin. Distribution results indicated that the hydrogel can significantly promote penetration and retention of the anti-inflammatory drug diclofenac.

Posterior diseases of the eye generally happen in the retina and choroid. Currently, a variety of approaches have been made to improve the delivery systems for the treatment of ocular posterior disease. The main purpose of all the efforts is to increase the controlled long-term delivery systems to reduce the frequency of injections, mainly including hydrogel, nanoparticles, nanoimplants and nanosized vesicles. For example, a drug-loaded nano unilamellar vesicle has shown an obvious reduction of the intraocular pressure, which can maintain the release of drug continuously over 120 days via a single subconjunctival injection. These promising results have facilitated the development of targeted delivery systems for eye-related therapy.
