Background of Fluorescent Anti-Nuclear Antibodies (FANA)

Fluorescent antinuclear antibody (FANA) test is commonly used in autoantibody detection for different autoimmune diseases and infectious diseases. Immunofluorescent microscopy is the gold standard method. In this method, the lab takes a slide coated with fixed, permeable cells and adds some of the patient's serum to that slide. The serum is allowed to incubate and is then washed off. FANA test is cool because the experimenter can tell not only if antibodies are present (by seeing green stuff on the cell nuclei) but also what kind of antibodies they might be (by seeing what pattern is present). Different patterns include homogeneous (diffuse) pattern, speckled pattern (random speckles all over the nucleus), nucleolar pattern (a few spots of green within the nucleus), rim or peripheral pattern (green around the periphery of the nucleus). The predictive value of a positive or negative FANA test result was calculated using different pretest probabilities based on clinical criteria.

The Role of Fluorescent Anti-Nuclear Antibodies (FANA) in Malaria

Malaria, a mosquito-borne infectious disease, affects humans and other animals caused by parasitic single-celled microorganisms belonging to the Plasmodium group. Antigen non-specific immunological derangements in malarial infections are well known for immunosuppression and autoantibody formation. In one case, it has been reported that on the first day of admission, 31.1% of the patients had positive FANA. The incidences of positive FANA rose progressively with times when the patients returned for the 2- and 4-weeks follow-ups after discharge, although most of their malaria was cured. Besides, FANA is significantly correlated with the initial hyper-IgM. Almost all of the positive FANA were the speckled type of nuclear staining. The antigen specificity of the speckled FANA was found not to be the double-stranded DNA or the extractable nuclear antigens.
