Background of Anti-E Antibody

Before the transfusion of the RBCs is applied, pre-transfusion testing such as ABO, Rh and Kell blood grouping with a negative cross-match is obligatory. Anti-E is the most common and important RBC alloantibody in the Chinese population. Being a less potent immunogen, clinical manifestations of anti-E alloimmunization are more variable. Anti-E alloimmunization can cause hemolytic disease of the fetus or newborn requiring prenatal intervention. In addition, anti-E antibodies are one of the most commonly implicated non-D Rhesus antibodies in the pathogenesis of hemolytic diseases of the newborn.

The Role of Anti-E Antibody in Thalassemia

To correct anemia in the β-thalassemia patients, regular blood transfusion regimen every 3-4 weeks is recommended that may lead to alloimmunization to erythrocyte antigens. Development of RBC alloantibodies and autoantibodies may complicate transfusion therapy. Some alloantibodies are hemolytic and may cause hemolytic transfusion reactions and limit the availability of compatible RBC units for transfusions. In numerous studies, the most frequently detected alloantibodies belong to Rh blood group system (52.17%). Individually, most frequent alloantibodies were anti-Kell (35%) and anti-E (17%).
