Background of Anti-gangliosides Antibodies

Ganglioside is a molecule harboring a glycosphingolipid (ceramide and oligosaccharide) with one or more sialic acids (such as n-acetylneuraminic acid) linked to the sugar chain. NeuNAc is the acetylated derivative of carbohydrate sialic acid, which helps gangliosides form the head group of ganglioside anion at pH 7, thus separating ganglioside from globosides. Anti-gangliosides antibodies reacting to self-gangliosides are found in autoimmune neuropathies. The natural autoantibodies were first found to react with cerebellar cells. These antibodies have the highest association with certain forms of Guillain-Barré syndrome. Currently, autoantigenic gangliosides include GD3, GM1, GQ3, and GT1. Anti-GD3 autoantibodies have been identified in an association with specific forms of Guillain-Barré syndrome. In vivo studies indicated that anti-GM1 and anti-GD3 autoantibodies can interfere with motor neuron function. The anti-GM1 autoantibodies levels are increased in patients with various forms of dementia, especially elevated in patients with prodromal diarrhea. The titers of anti-GM1 autoantibodies in other diseases such as rheumatoid arthritis, primary Sjögren's syndrome and systemic lupus erythematosus are also increased.

The Role of Anti-gangliosides Antibodies in Celiac Disease

Celiac disease (CD) is a T-cell-mediated, autoimmune permanent small bowel disorder triggered by ingested gluten proteins in genetically susceptible people. Characterized by intestinal malabsorption associated with villous atrophy of the small intestinal mucosa, the disease is improved after a gluten-free diet. Anti-ganglioside antibodies of IgG and IgM class have been reported in a series of adult CD patients with and without neurological symptoms, in patients with idiopathic neurological disorders and with autoimmune diseases. Moreover, the effect of a gluten-free diet on anti-ganglioside antibody positivity in CD patients with and without neurological involvement has been demonstrated. Therefore, anti-ganglioside antibodies may represent a new immunological marker to identify neurological impairment in patients with coeliac disease.
