Background of Anti-M2 Receptor Antibodies

M2 receptor is a muscarinic acetylcholine receptor encoded by the CHRM2 gene in humans. The M2 receptors play an important role in the heart by slowing the heart rate down to normal sinus rhythm. They also can reduce the contractile forces of the atrial cardiac muscle, and the conduction velocity of the atrioventricular node (AV node). Autoantibodies to M2 receptors are a class of abnormal antibodies produced by the immune system that attack M2 receptors mistakenly. Several reports have indicated that M2 receptor autoantibody, together with other related autoantibodies, linked to cardiac diseases such as myocarditis and dilated cardiomyopathy.

The Role of Anti-M2 Receptor Antibodies in Myocarditis

Myocarditis is an inflammation disease of the heart muscle, also referred to as inflammatory cardiomyopathy. The anti-M2 receptor antibodies have been demonstrated associated with the disease. Therefore, a synthetic peptide analogous of the human M2 receptor was used as an antigen to screen sera from patients with idiopathic dilated cardiomyopathy (DCM) and healthy blood donors. Obviously, the sera from patients with DCM can recognize the receptor peptide. Evidence also found that the antimuscarinic receptor-2 autoantibodies were associated with the anti-beta-adrenoceptor-1 autoantibodies in the myocarditis patients' sera. All of these data indicated that the anti-M2 receptor antibodies could be a useful biomarker during the diagnosis and treatment of myocarditis.
