CD154, also known as gp39, T-BAM, TRAP and Ly-62, is a CD40 ligand that belongs to the TNF family. It is a 39 kDa type II membrane glycoprotein firstly reported to be restricted to activate CD40 positive helper T lymphocytes. There are two forms of CD154 in lymphocytes: a cell bound full-length form (CD154, 30 kDa) and a truncated soluble form (sCD154, 18 kDa). The CD154 homotrimer is formed on many cell types such as basophils, B lymphocytes, activated T lymphocytes and so on. The CD40 receptor was initially authenticated and functionally characterized on B lymphocytes and acts a crucial role as an adjuster of T-cell-dependent B cell activation, proliferation and differentiation. The extensive spectrum of CD40–CD154 expression has developed a novel role of the CD40–CD154 dyad as a pivotal mediator of immunity and inflammation. It has indicated that the CD40–CD154 axis up regulates proinflammatory mediators, for example, the formation of proinflammatory cytokines (i.e. TNF-a, IL-6). These mediators have been proven to be increased in chronic heart failure (CHF), and possibly contribute to the development and progression of CHF.

CHF is a chronic progressive situation that influences the pumping capacity of heart muscles, particularly intends to the stage in which fluid builds up around the heart and results in inefficiently pumping of heart. Inflammation acts an important conducive role in the pathogenesis of CHF. CD154 Marker of Rheumatoid Arthritis

RA is a long-lasting inflammatory disorder that mainly affects joints. However, sometimes it also damages a great number of body systems, such as the skin, lungs, eyes, heart and blood vessels. Ligation of activated CD154+ CD4+ T cells to CD40 expressed on endothelial cells results in enhancing leucocyte margination and diapedesis due to the upregulation of some adhesion molecules. A study has revealed that improved expression of CD154 on CD4+ T cells in RA displayed prolonged and raised activation of CD4+ T lymphocytes and is related to active disease and probably an adverse prognosis.
