Introduction of Hepatitis A

Hepatitis A is the infectious disease of the liver caused by hepatitis A virus (HAV). As a member of Picornaviridae family, HAV is classified as a picornavirus with a genome of 7.5 kb. HAV is not enveloped and contains a single-stranded RNA packaged in a protein shell. There is only one HAV serotype, but multiple genotypes exist. Up to now, primates are the only natural hosts and the immunity after infection is lifelong. The interval between infection and symptoms is approximately 2 to 6 weeks and the possible symptoms include vomiting, nausea, jaundice, diarrhea, fever, and abdominal pain. There are few or no symptoms in many cases especially in the young, but the acute liver failure rarely occurs in the elderly. In general, the HAV is taken up in the gastrointestinal tract, then replicated in the liver, and finally excreted in bile. Thus, high concentrations of HAV can be detected in stool specimens. Furthermore, the IgM antibody to HAV can be served as the serologic marker of acute HAV infection to distinguish hepatitis A from other forms of viral hepatitis.

Epidemiology of Hepatitis A

The main transmission of hepatitis A includes the fecal-oral route, direct contact with an HAV-infected person, and ingestion of HAV-contaminated food or water. The transmission also occurs through exposure to HAV-contaminated blood or blood products, but not by exposure to saliva or urine. The median incubation period is nearly one month. As a food-borne disease transmitted by food or water, there is a high risk of infection with HAV in developing countries and regions with poor hygiene standards.
