Serpin H1

Serpin H1, also named HSP47, belongs to the serpin superfamily of serine proteinase inhibitors. It is encoded by the CBP2 gene located on chromosome 11q.13.5. The structure of Serpin H1 is characterized by a signal sequence at the N-terminal, two N-glycosylation sites and an ER retention signal at the C-terminal. Serpin H1 is a molecular chaperon that is essential for the proper folding and secretion of collagen proteins. The mechanisms of this protein in the maturation and secretion include inhibition of local unfolding of procollagen and inhibition of aggregation after triple-helix formation. It has been found to be upregulated in many fibrotic diseases, including liver fibrosis and pulmonary fibrosis. Besides, Serpin H1 is overexpressed in gastric, pancreatic cancers and glioblastoma.

Roles of Serpin H1 in Cancers

Serpin H1 is increased in many cancers, such as breast cancer and clear renal cell carcinoma (ccRCC), as well as cholangiocellular carcinoma. In breast cancer, Serpin H1 is involved in cancer progression and poor clinical outcome. It was illustrated that inhibition of Serpin H1 expression dramatically hampered tumor growth with the reduced deposition of collagen and fibronectin. Therefore, Serpin H1 promotes breast cancer progression by regulating ECM. And in renal cell cancer, it has been revealed that Serpin H1 was overexpressed in a grade-dependent way. And the possible mechanism that Serpin H1 affects the renal cancer patients is through regulating the expression of EMT-related proteins.

Serpin H1 Marker for Cholangiocarcinoma Diagnosis

Cholangiocarcinoma is a tumor that arises from the cholangiocyte and the epithelial cells and can be classified as intrahepatic, extrahepatic and hilar. This kind of tumor consists of the great amount of fibrous stroma. Importantly, Serpin H1 is highly expressed in cholangiocarcinoma. As a collagen-specific molecular chaperon, Serpin H1 participates in processing, glycosylation, and secretion of collagen. The upregulated expression of Serpin H1 predicts a strong collagen synthesis and fibrotic changes, thus, Serpin H1 can serve as an essential diagnostic biomarker for cholangiocarcinoma.
