Apolipoprotein CI

Apolipoprotein CI (Apo CI) is one of the most positively charged proteins in the human body. Human Apo CI is a lipoprotein with a length of 57 amino acids. It is a highly exchangeable protein and can dissociate from the VLDL surface to rapidly interact with HDLs, which are the main carriers of Apo CI in normolipidemic plasma. In hypertriglyceridemic states, in particular when VLDL secretion by the liver is increased, substantial amounts of Apo CI are associated with the VLDL fraction. As a result of its dual localization in HDL and VLDL, Apo CI has been found to play a significant role in the metabolism of these two classes of lipoproteins. On the one hand, Apo CI was found to stimulate the hepatic production of VLDL, to inhibit the hydrolysis of VLDL by LPL, and recognition of VLDL by cellular receptors. In vivo studies in animal models support the hypothesis that Apo CI plays a complex and significant role in both the accumulation of VLDL particles in the bloodstream and the reduction in their cholesteryl ester content relative to triglycerides. This effect of Apo CI on VLDL clearance has been shown to be of physiological relevance in humans. On the other hand, purified human Apo CI is a potent inhibitor of cholesteryl ester transfer protein (CETP) activity when associated with HDL, and as such it has the ability to reduce the net mass transfer of cholesteryl esters from HDL toward VLDL.

The Role of Apolipoprotein CI in Sepsis

Apo CI plays a role in inflammation and infection. Apo CI has been revealed to be associated with the production of VLDL. And VLDL can protect organisms against bacterial infection. Moreover, Apo CI can bind lipopolysaccharide (LPS), the main toxic component of Gram-negative bacteria and possess a pro-inflammatory property. By enhancing the biological response to LPS and Gram-negative bacteria, Apo CI dose-dependently enhances the anti-bacterial attack and protects against intrapulmonal Klebsiella pneumoniae-induced sepsis. Furthermore, a study shows that the high plasma levels of Apo CI are less susceptibility to infection-related mortality during follow-up, independent of plasma lipid levels. And the higher plasma Apo CI levels are detected in survivors of severe sepsis compared to non-survivors, again independent of lipid levels.
