Introduction of TREM-1

Triggering receptor expressed on myeloid cells 1 (TREM-1) is a protein encoded by the human TREM1 gene. As a member of the Ig superfamily, it is a 30-kDa glycoprotein receptor containing an ectodomain of the Ig-like V-type, a transmembrane part, and a short intracytoplasmic domain. Generally, TREM-1 is expressed on neutrophils and monocytes and plays an important role in systemic infections. Engagement of TREM-1 on the surface of the cell membrane leads to activation of a cascade of intracellular events that result in inflammatory response including the production of cytokines, chemokines, and reactive oxygen species. Furthermore, TREM-1 signaling leads to degranulation of neutrophils and increased phagocytosis.

TREM-1 Marker of Sepsis

Severe sepsis is a major cause of morbidity and mortality in humans for it can result in activation of various immunological mechanisms and coagulation cascades that are detrimental to the host. Studies have shown that the expression of membrane-bound TREM-1 by effector cells is dramatically increased in skin, biological fluids and tissues infected by Gram-positive and Gram-negative bacteria and fungi, especially in patients with sepsis. Furthermore, the soluble form of this receptor (sTREM-1) which can be measured in biological fluid also increases. In this case, the rapid assessment of sTREM-1 concentration could be used as a valuable tool for the diagnosis of infection, particularly with regard to its plasma levels in sepsis.
