Introduction of Leptin

Leptin, which is described as the hormone of energy expenditure, belongs to the family of long-chain helical cytokines, including leukemia inhibitory factor, ciliary neurotrophic factor (CNTF), and human growth hormone, based on its crystal structure. The lep gene where leptin derives from is highly preserved across mammals, and leptin orthologs exist in amphibians, reptiles, and fish with considerable divergence in primary amino acid sequences. As a hormone that has the ability to regulate energy balance by inhibiting hunger, leptin is predominantly produced and secreted from adipose tissue into the circulation. Circulating leptin levels positively reflect adipose tissue size, and communicate energy storage status to the brain. In addition, the expression and secretion of leptin can be regulated by many factors, such as inflammatory cytokines, glucocorticoids, and insulin.

Leptin Marker of Sepsis

Sepsis is a systemic inflammatory reaction which is triggered by an infective agent, such as bacteria, viruses, fungi or parasites. Severe sepsis may result in systemic inflammation, multi-organ failure, and septic shock. Infections and sepsis are accompanied by clinical signs such as leukocytosis, changes in body temperature and the development of tachycardia. However, these classic indicators of systemic inflammation are neither sensitive nor specific for sepsis. Therefore, early markers are useful for the diagnosis and treatment of sepsis. In recent years, studies have suggested that the serum leptin concentrations of patients with sepsis were significantly higher than that of normal people. What's more, it has also been demonstrated that leptin, in combination with other independent factors, such as temperature, platelet and WBCs and heart rate, may be an effective diagnostic model of sepsis. Under this condition, the serum level of leptin can be used as a specific marker for the diagnosis of sepsis.
