Endocan

Endocan, also called endothelial cell-specific molecule 1, is encoded by the ESM1 gene in human. It is a secreted protein mainly expressed in the endothelial cells of human lung and kidney tissues. Endocan is identified as a dermatan sulfate proteoglycan which is regulated by cytokines, indicating its function in endothelium-dependent pathological disorders. Proteoglycans (PGs) are proteins that have glycosaminoglycan chains covalently linked to the core of these proteins. Several types of PG have been found in extracellular matrices (ECM) of connective tissues in which PGs provide their main biological action of structural support. Endocan is a new PG secreted by vascular endothelium, which has distinct biological functions. Current studies demonstrate that endocan may have the key role in inflammation, healing, and tumorigenesis.

Diagnostic Utility of Endocan in Sepsis

On account of its involvement in the regulation of cellular behavior, the potential significance of endocan as a biomarker for sepsis is increasingly being explored. Sepsis has related endothelial dysfunction ranging from vasodilation, edema to coagulopathy, ischemia, and organ failure. Because the inflammatory mediators, such as IL-1 and TNF-α, induce endocan expression, blood endocan levels may closely reflect the existence and severity of inflammation as well as the response to therapy. A recent study has demonstrated the increased blood levels of cathepsin G-cleaved endocan in patients with sepsis. The cathepsin G-cleaved endocan is a 14 kDa circulating protein (p14), which is identified as the fragment of endocan specifically cleaved by cathepsin G, a neutrophil-derived serine protease. Another study indicates that blood level of circulating endocan is associated with the severity of sepsis and also reflected the prognosis of this disease. Above all, endocan is now regarded as a novel blood-and tissue-based biomarker for sepsis.
