The RHCE gene encodes one of the core proteins of the Rh blood group system. RhCE is a non-glycosylated polypeptide with a molecular weight of approximately 45–50 kDa localized to the erythrocyte membrane. This protein forms a complex with the RhAG protein, jointly contributing to the structural organization of the Rh blood group complex and the integrity of the red blood cell membrane. Allelic variations of the RHCE gene directly determine the expression of the C, c, E, and e antigens. Their absence or alteration can contribute to Rh deficiency phenotypes associated with hemolytic anemia and abnormal red blood cell morphology. In clinical transfusion medicine, RHCE genotyping is of great significance for preventing hemolytic disease of the fetus and newborn and hemolytic transfusion reactions. Moreover, altered expression of Rh blood group antigens has been investigated in immune-mediated hemolytic conditions, making RHCE an important target in blood group immunology and transfusion safety research.
