Serine protease inhibitor A3 (also known as α1- anticchymotrypsin) is a secreted serine protease inhibitor encoded by SERPINA3 gene, which is mainly synthesized by liver parenchymal cells and activated inflammatory myeloid cells and released into the blood circulation, and can be stored in neutrophil cytoplasmic granules, which is widely involved in the steady-state regulation of systemic inflammation. Different from serine inhibitors such as A1AT, which have broad-spectrum protease inhibition, SERPINA3 has highly selective inhibitory activity and specifically targets chymotrypsin-like serine proteases such as cathepsin G and mast cell chymotrypsin. Under the physiological steady state of the body, SERPINA3 expressed in circulation can neutralize a small amount of protease leaked from granulocytes, avoid non-specific degradation of extracellular matrix and maintain the integrity of tissue structure. Under the stress of acute infection, the liver greatly upregulates SERPINA3 secretion through the acute reaction mediated by IL-6, which effectively restricts the excessive release of neutrophil protease and reduces the tissue damage caused by systemic inflammation. However, long-term chronic liver injury can continuously induce the increase of SERPINA3 expression, making it a sensitive clinical marker reflecting the progress of liver fibrosis. Other serine protease inhibitors can't compensate for the specific chymotrypsin inhibition function of SERPINA3. The deletion of SERPINA3 will significantly aggravate the organ damage mediated by inflammation. The high expression of SERPINA3 in serum can stably indicate the degree of inflammation and fibrosis, which is an irreplaceable core target for protein mechanism research and targeted screening of liver fibrosis in acute phase.
SERPINA3, as a secreted serine protease inhibitor, mainly plays a biological role in interstitial tissues and circulating body fluids, and irreversibly captures serine proteases of chymotrypsin family by means of conservative reaction center loop (RCL) structure, forming a stable inactive complex, thus terminating the protease-mediated tissue injury pathway. Its narrow and specific protease recognition spectrum forms a clear functional differentiation with broad-spectrum A1AT, and constructs the body's exclusive anti-inflammatory defense system in acute phase. SERPINA3-mediated protease neutralization effect can closely couple the acute stress signal of liver with the protective mechanism of tissue matrix, and dynamically regulate the balance between physiological inflammation clearance and pathological organ scar repair according to the duration of injury. This molecule is widely involved in many physiological and pathological processes, such as systemic acute inflammation regression, chronic liver fibrosis evolution and inflammatory degeneration of central tissue. The loss of SERPINA3 function will greatly amplify the protease activation effect stimulated by inflammation and aggravate the structural damage of tissues. To sum up, SERPINA3 is the key target for the mechanism analysis of serine protease inhibitors and the research and development of targeted therapy for liver diseases.
