Interleukin 1 receptor like 1 (IL1RL1), also known as ST2, is a member of the interleukin-1 receptor family and serves as the primary receptor for the alarmin cytokine IL-33. The IL1RL1 gene gives rise to multiple isoforms, most notably the membrane-bound signaling receptor ST2L and the soluble isoform sST2. ST2L is a single-pass transmembrane glycoprotein containing extracellular immunoglobulin-like domains, a transmembrane region, and an intracellular Toll/IL-1 receptor (TIR) domain. It is expressed by a broad range of immune and tissue-resident cell populations involved in type 2 immunity, including group 2 innate lymphoid cells, mast cells, eosinophils, Th2 cells, epithelial cells, and stromal cells. In contrast, sST2 lacks the transmembrane and intracellular signaling regions and functions primarily as a soluble decoy receptor that can sequester IL-33 and limit its availability for ST2L-dependent signaling.
IL1RL1 mediates IL-33 signaling through a stepwise receptor assembly mechanism. IL-33 first binds directly to membrane-bound ST2L, forming an IL-33-ST2L binary complex that subsequently recruits IL-1 receptor accessory protein (IL1RAP/IL-1RAcP) to generate the signaling-competent heterodimeric receptor complex. Juxtaposition of the intracellular TIR domains promotes recruitment of adaptor and signaling proteins, including MyD88, IRAK family kinases, and TRAF6, leading to activation of downstream NF-κB and MAPK pathways and the induction of type 2 inflammatory gene programs. Through this pathway, IL1RL1 regulates alarmin sensing, innate and adaptive type 2 immune responses, mucosal inflammation, tissue repair, and allergic immune activity. The balance between signaling-competent ST2L and soluble sST2 provides an additional level of regulation over IL-33 bioavailability and signaling intensity. Dysregulated IL-33–IL1RL1 signaling has been associated with excessive type 2 inflammation and a range of chronic allergic and inflammatory conditions, making IL1RL1 an important research target for investigating alarmin biology, mucosal immunity, and type 2 inflammatory mechanisms.
