HighlightsAntagonists stem from Latin antagonista and Greek antagonistes, which means competitor, rival or opponent. In medicine, an antagonistic antibody works against the drug and blocks the immune response, whereas an agonistic antibody binds to a receptor site and induces a cascade response. Immunity responses are under the control of extracellular molecules, which exert functions in receptors present on the cell surface of the immune system and thereby are amenable to be functionally regulated by monoclonal antibodies (mAbs). Some of these mechanistic bases are activating and decide whether the response continues, while others play the role of powerful repressors. It is known that the antagonist is a key player in the human body and in clinical practices and performs on such repressors leading to enhanced immune responses. Receptor antagonists are inhibitors of receptor activity. They mimic ligands that are coupled to a receptor and prevent receptor activation by a natural ligand. And though antagonists help in binding, they would not alter the activity of receptors.
A series of therapeutic agents based on antagonistic antibodies are under pharmacological development. One of them directed at the cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4) inhibitory receptor, namely ipilimumab, has been approved for the treatment with metastatic melanoma. The list of antagonist agents acting on repressors is currently being developed, which includes anti-PD-1, anti-PD-L1 (B7-H1), anti-CTLA-4, anti-KIR, and anti-TGF-β. A promising preclinical pipeline suggests that many other active targets are also being tested in patients in the near future. All of these antibodies have been studied as conventional monoclonal immunoglobulins, but other engineered antibody formats are undergoing preclinical scrutiny.

The generation and maintenance of immune responses are controlled by both co-inhibitory and co-stimulatory signaling through T cell receptors, many of which belong to the tumor necrosis factor (TNF) receptor superfamily or the immunoglobulin-like superfamily.
