  • Introduction
  • Claudin-18 (CLDN18), a member of the claudin family, is a four-pass transmembrane protein with two extracellular loops and cytoplasmic N- and C-tails, found in tight junctions. CLDN18 regulates paracellular permeability, cell polarity, and signaling. Deficiency in CLDN18 has been linked to conditions such as atrophic gastritis, spasmolytic polypeptide-expressing metaplasia (SPEM), and asthma. The lung-specific isoform, CLDN18.1, influences alveolar epithelial type II (AT2) cell proliferation and organ size. Restoring CLDN18.1 expression in human lung cancer cells has been shown to suppress proliferation by inhibiting the IGF-1R/AKT signaling axis. While CLDN18 is not expressed in other healthy tissues, the gastric mucosa-specific isoform, CLDN18.2, is highly expressed in primary gastric cancers. CLDN18.2, present on the surface of epithelial tumor cells, is a promising target for antibody and CAR-T therapies currently under development.
