Background

CLDN18 is a tissue-specific tight junction transmembrane protein encoded by CLDN18 gene, which is mainly located in the tight junction area at the top of gastric epithelial cells, expressed at a low level in alveolar epithelium, and distributed in the junction between epithelial cells as a whole. Different from other members of the tight junction protein family, CLDN18 mainly contains two splicing isomers, CLDN18.1 and CLDN18.2. CLDN18.2 has strong tissue specificity of gastric mucosa, and it is often abnormally highly expressed in gastric cancer cells. It can regulate the transmembrane transport process of ions and small molecules in epithelial monolayer structure by constructing selective paracellular permeability barrier.

In the physiological steady state of gastric mucosa, the basic expression of CLDN18 can maintain the integrity of gastric epithelial barrier structure, effectively block gastric acid reflux and avoid inflammatory damage of gastric mucosa. In the process of malignant transformation of cells, the abnormal up-regulation of CLDN18.2 will reshape the tight junction structure on the surface of tumor cells, and then help tumor cells to proliferate, matrix invasion and distant metastasis. Compared with the ubiquitously expressed tight junction barrier protein, the gastric tissue-specific expression characteristics of CLDN18.2 are irreplaceable, and other tight junction protein homologous subtypes cannot completely compensate their physiological and pathological functions, which makes CLDN18.2 a key molecule connecting epithelial tight junction assembly, gastric mucosal defense mechanism and malignant progression pathway of gastric cancer.

The loss of CLDN18 function will destroy the integrity of gastric mucosal barrier and induce gastritis damage. The overexpression of CLDN18.2 can be used as a specific tumor marker of gastric cancer. Based on the above characteristics, CLDN18 has become the core target of epithelial tight junction biology research and targeted therapy of gastric cancer antibody drug conjugate (ADC). From the molecular mechanism, CLDN18 is embedded in the lipid bilayer at the top of epithelial cells, which plays a role in the construction of paracellular barrier. The protein has four conserved transmembrane helix structures and extracellular domain, which can form tightly connected dimers between cells by homologous pairing, and its conserved extracellular domain is the core structure that mediates its own homologous binding, which is also an important structural basis for its differentiation from other proteins in the family and its tissue expression specificity. CLDN18-mediated tight junction assembly process can maintain the adhesion stability between epithelial cells, accurately regulate the permeability of gastric mucosa, and balance the isolation efficiency of gastric acid and the susceptibility to mucosal inflammation according to the physiological state of tissues.The protein is widely involved in many biological processes, such as the maintenance of gastric mucosal barrier, the regulation of alveolar fluid homeostasis and the invasion and progression of gastric cancer. Its insufficient expression will lead to the disorder of epithelial tight junction network and significantly increase the risk of gastric mucosal injury. To sum up, CLDN18 has important research value and application prospect in the research of tight junction protein mechanism and the screening of targeted therapy for gastric cancer.
