Background

Osteoarthritis is a common degenerative disease of articular cartilage. Its pathological changes often include cartilage wear, synovial inflammation, and subchondral bone lesions. It is characterized by deformation, destruction and bone hyperplasia of articular cartilage. The clinical manifestations are slow-developing joint pain, stiffness, swelling, and deformity, which makes the patient difficult lifting heavy objects, causing great pain to the patient and seriously affecting the quality of life of the patient. According to the World Health Organization, approximately 355 million people worldwide have osteoarthritis. In the past, osteoarthritis was often thought to be a kind of geriatric disease. In fact, osteoarthritis has shown a trend of rejuvenation in recent years: the office workers who sit still in front of the computer, the drivers who grasp the steering wheel for a long time, young women who love to wear high heels and young people who have long pressed the mobile phone keyboard are prone to osteoarthritis. In the late stage of osteoarthritis, the patient's activities are gradually limited, and even the loss of joint function which may lead to disability, so it is also called "disability killer". Regrettably, there is still no means to cure osteoarthritis.

haMPC-based Vaccine Brings Promise in Treat Osteoarthritis

Conventional methods of treating osteoarthritis include taking anti-inflammatory analgesics, intra-articular injections of sodium hyaluronate, physical therapy, surgery, and the like. The only recommended osteoarthritis treatment for the American College of Orthopaedic Surgeons (AAOS) guidelines is non-steroidal anti-inflammatory analgesics, but this class of drugs does not delay the progression of articular cartilage wear and osteoarthritis. Articular cartilage has no blood vessels and nerves, and damaged cartilage tissue is difficult to repair itself. Mesenchymal stem cells (MSC) have good self-replication and multi-directional differentiation potential, have the potential to cure a variety of degenerative diseases, and studies have shown that the bone marrow mesenchymal stem cells (BMSC), human adipose-derived mesenchymal progenitor cells (haMPCs), synovial-derived stem cells, etc. can effectively treat OA. Among them, haMPCs have attracted much attention due to their rich sources, convenient materials and high content in tissues.

It is currently believed that the therapeutic effects of haMPCs on OA are mainly achieved through these three effects: 1) differentiation into chondrocytes, repair of damaged cartilage tissue; 2) secretion of anti-inflammatory factors and trophic factors to promote proliferation of cartilage precursor cells, and inhibit destruction of cartilage tissue and reduce cartilage damage through suppression of cartilage inflammation, apoptosis, hypertrophy, fibrosis, and dedifferentiation of cells; 3) immunomodulatory effect, which is to inhibit the release of inflammatory factors and reduce OA inflammation by secreting anti-inflammatory factors to inhibit the activity of macrophages in the joint cavity.
