Over the last decade, it has become clear that human papillomavirus (HPV) not only causes genital and anal cancers but also causes a subset of head and neck squamous cell carcinoma (HNSCC). In addition to the estimated ~492,800 cervical cancers caused worldwide by HPV each year, HPV also causes an estimated ~30,000 oropharyngeal cancers, HPV is detected in ~25% of all HNSCC, and the majority of these HPV-associated HNSCC are oropharyngeal (tonsillar and base of tongue) squamous cell cancers. As more and more information about the role of infection in non-cervical diseases is amassed, additional questions about whether prophylactic human papillomavirus vaccines will effectively prevent these conditions are raised. In fact, HPV is now the major cause of oropharyngeal cancer in developed countries, detected in 45–90% of cases. HPV has also been detected in a smaller subset of laryngeal and oral cavity cancers.

Oral Cancer

Oral cancer is a type of head and neck cancer that grows in any cancerous tissue located in the mouth. Recent studies have shown a strong relationship between human papillomavirus (HPV) infection and head and neck cancer, especially oropharyngeal squamous cell carcinoma (OPSCC) and oral squamous cell carcinoma (OSCC). The incidence of HPV-related OSCC appears to be on the rise while HPV-unrelated OSCC tends to have stabilized in the past decades.

Nasopharynx Cancer

Nasopharynx cancer, also known as nasopharyngeal carcinoma (NPC), is the most common form of cancer originating in the nasopharynx. Viral, diet and genetic factors are related to their etiology. The causes, occurrence, clinical manifestations and treatment are significantly different with nasopharynx cancer and other head and neck cancer. Nasopharyngeal carcinoma is associated with the Epstein-Barr virus (EBV) and characterized by peritumoral immune infiltrate. Therapeutic cancer vaccines for NPC have historically targeted EBV antigens.

Oropharyngeal Cancer

Oropharyngeal cancer (OPC) is a cancer arising from the throat (oropharynx) tissues. HPV-positive OPC is related to human papillomavirus infection while HPV-negative OPC is usually associated with alcohol or tobacco use. People with HPV-positive oropharyngeal cancer tend to have higher survival rates. Approximately 70-90% of newly diagnosed OPCs in the United States are HPV-related. These HPV-related cancers are often seen in younger, healthier patients, many of whom are non-smokers. In the face of this changing epidemiology, there is increasing interest in the potential impact of HPV vaccines on the prevention of OPC.

Laryngeal Cancer

Laryngeal cancer is a type of cancer arising from the skin of laryngeal, mostly squamous cell carcinoma. Smoking still remains the most common cause of laryngeal cancer but the link between laryngeal cancer and HPV is an area of ongoing medical research. There is growing evidence that HPV plays a role in some types of laryngeal cancer.
