Background of Inflammation Treatment

Normally, inflammation is a kind of immune defense that protects the body from the stimulation of pathogens, damaged cells, or irritants, which is a complex biological response. The main functions of inflammation are to clear out necrotic cells and tissues and initiate tissue repair. However, the abnormal inflammatory response can result in a series of human diseases and disorders, such as immune-related diseases, cancers, atherosclerosis, and ischemic heart disease. Inflammatory diseases are generally treated with two categories of agents, namely steroidal anti-inflammatory drugs (corticosteroids) and non-steroidal anti-inflammatory drugs.

Currently, steroidal anti-inflammatory drugs are a class of steroid hormones that are lipophilic with poor solubility in water, while non-steroidal anti-inflammatory drugs are a heterogeneous group of compounds such as aspirin and ibuprofen, which mostly are non-specific and long-term causing some side effects including gastrointestinal diseases and kidney disease. Therefore, the most challenging inflammatory treatment problem for pharmaceutical scientists is the poor water solubility and side effects.

Lipid-Based Drug Delivery System Applied for Inflammation Treatment

Lipid-based drug delivery system has received increasing attention in applications for the treatment of inflammatory diseases due to numerous noted advantages. Numerous studies have been conducted to develop novel formulations capable of promoting the therapeutic effect of anti-inflammatory drugs and minimizing side effects using lipid-based drug carriers, such as liposomes, exosomes, lipid-based nanoparticles, niosomes, ethosomes and so forth. In terms of steroidal anti-inflammatory drugs, lipid-based drug delivery system is a good approach to improve their low aqueous solubility, meanwhile, steroidal drugs dispersion in lipid matrix can sustain the drug release. Solid lipid nanoparticles and nanostructured lipid carriers are more suitable to develop topical anti-inflammatory drugs since the proper sizes and a thin film over the skin surface formed by these lipid structures can increase drug penetration. Non-steroidal anti-inflammatory drugs encapsulated by lipid microspheres or liposomes are more effective than conventional formulations.
