What is Vaginal Candidiasis?

Vaginal candidiasis, which is also called a vaginal yeast infection, vulvovaginal candidiasis, or candidal vaginitis, is a mucosal infection that affects plenty of otherwise healthy women of childbearing age. It is an opportunistic fungal infection that usually caused by Candida albicans but occasionally be caused by other Candida sp. or yeasts. C. albicans is a commensal organism of the gastrointestinal and reproductive tracts, which causes 85~90% of cases of vaginal candidiasis. A healthy vagina contains bacteria and some yeast cells. Generally, Candida lives in the mouth, throat, gut, as well as the vagina of human bodies, without causing any problems. However, the excessive growth of Candida in the vagina would lead to irritation and result in vaginal candidiasis.

Symptoms of Vaginal Candidiasis

Although most vaginal candidiasis is mild, some patients can develop moderate or severe infections. The symptoms of vaginal candidiasis include:

  • Itching and irritation in the vagina and the tissues at the vaginal vulva;
  • A burning sensation, especially during urination or intercourse;
  • Redness and swelling of the vulva;
  • Vaginal rash;
  • Vaginal pain and soreness;
  • Cracks in the vaginal wall;
  • Watery vaginal discharge;
  • Abnormal vaginal discharge, e.g. thick, white, odor-free vaginal discharge with a cottage cheese appearance.

Diagnosis of Vaginal Candidiasis

Vaginal candidiasis is simple to diagnose. A laboratory examination is usually needed to diagnose vaginal candidiasis since the symptoms are similar to those of other types of vaginal infections. A small sample of vaginal discharge would be collected to be examined under a microscope or sent to a laboratory for a fungal culture. The presence of fungi can also be diagnosed through antigen tests. However, a positive fungal culture does not always imply that Candida is the cause of the symptoms because some women may have Candida in the vagina without having any symptoms.

Treatment of Vaginal Candidiasis

Generally, vaginal candidiasis is treated with antifungal drugs. Antifungal therapy is useful for individual attacks of vaginal candidiasis but does not prevent recurrence, and antifungal drug resistance does not lead to recurrence. For most infections of vaginal candidiasis, the therapeutic method is with intravaginal or oral antifungal drugs. For more severe situations, infections that don't get better, or keep recurring after getting better, other treatments may be needed.
