What is Fungal Keratitis?

Fungal keratitis (FK) is a form of keratitis that caused by fungi. Of the organisms that cause keratitis, fungi remain one of the most elusive and challenging organisms to diagnose and treat. It has also been shown that infection with FK can be more virulent and damaging compared to that of a bacterial origin. Commonly, FK occurs in the tropical and subtropical parts of the world, such as in south India, where fungi cause up to 47% of keratitis cases and where filamentous fungi, such as Fusarium and Aspergillus, predominate. Moreover, FK also occurs in the more temperate areas of the world but more often caused by yeasts, such as Candida.

Symptoms of FK

Patients with fungal keratitis usually report a sudden onset of pain. In addition, common symptoms of fungal keratitis include:

  • Eye redness
  • Blurred vision
  • Sensitivity to light
  • Excessive tearing
  • Eye discharge
  • Foreign-body sensation

Diagnosis of FK

Currently, tissue sampling and culture continues to be an imperative utility in the diagnosis of fungal keratitis. The best diagnostic specimen is material from the affected part of the cornea collected by scraping any area of ulceration or biopsy when the corneal epithelium is intact and the infection is within the corneal stroma. The material should be collected from the base and edges of the ulcerated part of the cornea.

Treatment of FK

During the past years, many antifungal agents have been developed for the treatment of FK. Generally, topical antifungal therapy is essential, with the severity of the disease dictating how frequently eye drops are administered. For superficial lesions caused by filamentous fungi, topical natamycin or amphotericin B are usually used. However, each antifungal agent has its benefits and limitations, and no one agent has emerged as the best and most cost-effective agent. Therefore, it is of great urgency to discovery more effective antifungal drugs to treat FK.
